Abstract
Prvi slučaj autizma zabilježen u literaturi bio je divlji dječak Viktor iz Aveyrona (1799. godine) koji se ponašao poput životinje te nije pokazivao nikakve oblike civiliziranog ponašanja. Pojam autizam prvi je spomenuo Eugen Bleuer 1911. godine kako bi opisao jedan od glavnih simptoma shizofrenije. Kanner je 1943. godine upotrijebio pojam „Autistični poremećaj afektivnih veza“ i opisao obilježja jedanaestero djece, nad kojima je proveo istraživanje, koja od rođenja imaju nemogućnost uspostavljanja kontakta s ljudima. Ne znajući za Kannerov rad, Hans Asperger 1944. godine opisao je slučajeve djece pod naslovom „Autistična psihopatija u dječjoj dobi“. On je smatrao da su ta djeca ograničena u stvaranju odnosa, motorički nespretna, ali ipak inteligentna, odnosno smatrao je da imaju poremećaj ličnosti. Klasifikaciju i dijagnosticiranje poremećaja obavlja tim stručnjaka koristeći se dvaju, trenutno najrelevantnija dijagnostička i statička priručnika za klasifikaciju duševnih poremećaja, a to su MKB- 10 (Međunarodna klasifikacija psihičkih poremećaja, deseto izdanje) i DSM- V (Dijagnostički i statički priručnik za duševne poremećaje, peto izdanje). Prateći DSM- IV, u MKB- 10 koristi se pojam pervazivni razvojni poremećaji koji ima osam vrsta poremećaja u podkategoriji. Svi dosadašnji poremećaji koji su se povezivali s autizmom prema DSM- V spadaju u jednu cjelinu, a to je Poremećaj iz spektra autizma. Kao glavna obilježja PAS- a mogu se izdvojiti deficiti u socijalnoj komunikaciji, socijalnim interakcijama te stereotipne radnje. Kod djece s PAS- om dolazi do poremećaja u teoriji uma kao i poremećaja u izvršnim funkcijama. PAS je poremećaj koji je u komorbiditetu mnogim poremećajima kao što su: Rettov poremećaj, ADHD, dispraksija, specifične teškoće u učenju, epilepsija i dr. Jako je važno prepoznati simptome poremećaja kako bi se na vrijeme mogle primjeniti metode, usluge i načini intervencije. Jedne od tih metoda su: ABA, TEACCH, floor- time te program ravnoteže mozga i program Son- Rise. Djeca s PAS- om imaju mogućnost inkluzivnog obrazovanja i uključivanja u vrtićku skupinu kako bi se cjelovito razvijali i kako bi razvijali sve svoje potencijale. Uključivanjem djeteta u vrtićku skupinu, moguće je da radi još jedan odgojitelj ili stručnjak edukacijsko-rehabilitacijskog profila, a uloga odgajatelja je ključna.
Abstract (english)
Victor, The Wild Boy of Aveyron, was the first recorded case of autism in literature (1799). He behaved like a wild animal, lacking any traits of civilised behaviour. The term autism was first used by psychiatrist Eugen Bleuler in 1908 in order to describe a schizophrenic patient who had withdrawn into his own world. Leo Kanner described eleven children he had studied in a 1943 paper entitled "Autistic Disturbances of Affective Contact". All of them had had the inability to make contact with other people. Not knowing about his research, Hans Asperger published his 1944 paper entitled "Autistic Psychopathy in Childhood". He believed the deficts autistic children have prevent normal social intercourse, such clidren are clumsy but still intelligent, they, namely, have a mental disorder. A team of experts does the clasification and the diagnose of disorders using two most revelant diagnostic and statistic manuals of mental disorders, ICD-10 (The international classification of mental diseases, edition 10) and DSM-V (Diagnostic and statistical manual of mental diseases, edition 5). Relying on DSM-IV, ICD-10 uses the term pervasive developmental disorder that refers to eight separate disorders. All previous disorders that have to do with autism are, according to DSM-V, put in one group called autism spectrum disorder. It is characterised by socially and emotionally inapropriate behaviour, difficulty interacting with peers and the repetitive patterns of behaviour. Within such children there is a disorder in the theory of mind as well as the disorder of executive functions. ASD is in co-morbidity with other disorders such as Rett Syndrome, ADHD, dyspraxia, specific learning difficulties, epilepsy and others. It is very important to diagnose the symptoms of disorders in order to secure proper treatment options. Such therapies are ABA, TEACCH, floor-time, brain balance program and Son-Rise program. Children with ASD are educated in mainstream kindergarten classrooms in order to achieve full development. In such a group there can be one more educator or an education-rehabilitation expert, but it is the educator herself/himself that has the major role.
