Materijali i metode: U istraživanju su sudjelovala 804 ispitanika kojima je provedena cjelonoćna polisomnografija u Centru za medicinu spavanja Split. Ispitivanu skupinu činili su pacijenti kojima je dijagnosticirana opstrukcijska apneja tijekom spavanja temeljem vrijednosti Apneja Hipopneja Indeksa (AHI) koja je iznosila ≥5, dok su drugu skupinu ispitanika činili ispitanici kojima temeljem cjelonoćne polisomnografije nije dijagnosticirana opstrukcijska apneja tijekom spavanja, tj. ispitanici kojima je vrijednost AHI iznosila <5. U istraživanju su sudjelovali ispitanici stariji od 18 godina. Ispitanici su, osim zapisa cjelonoćne polisomnografije, imali ispunjene upitnike za procjenu prekomjerne dnevne pospanosti (Epworthova ljestvica pospanosti). Od ukupno 804 ispitanika, 242 ispitanika pregledao je specijalist otorinolaringologije prije cjelonoćnog polisomnografskog snimanja, te odredio stupanj opstrukcije gornjeg dišnog puta procijenjen prema Mallampati klasifikaciji te veličinu i položaj jezika procijenjen prema Friedman klasifikaciji.
Rezultati: Rezultati ovog istraživanja pokazali su da ispitanici kojima je dijagnosticirana opstrukcijska apneja tijekom spavanja imaju veću prekomjernu dnevnu pospanost nego ispitanici kojima nije dijagnosticirana opstrukcijska apneja tijekom spavanja (7,7 ± 4,9 vs 6,7 ± 4,6, P = 0,013). Nadalje, rezultati istraživanja pokazali su kako u populaciji pacijenata s OSA bolju korelaciju s vrijednošću AHI ima razina dnevne pospanosti procijenjena prema Epworthovoj ljestvici pospanosti (ESS) nego stupanj opstrukcije gornjeg dišnog puta procijenjen po Mallampatiju ili veličina jezika klasificirana po Friedmanu.
Zaključak: Temeljem rezultata ovog istraživanja utvrđeno je da veći stupanj opstrukcije gornjeg dišnog puta i veličine i položaja jezika, procijenjenih prema Mallampati i Friedman klasifikaciji, nije nužno povezan s većim stupnjem opstrukcijske apneje tijekom spavanja, odnosno stupanj IV prema Mallampati klasifikaciji (raspon I-IV) ne ukazuje s velikom vjerojatnošću na veći stupanj OSA, kao što i stupanj I ne može sa sigurnošću isključiti dijagnozu OSA.
Abstract (english) Objectives: The aim of this study was to establish the correlation between obstruction of the upper respiratory tract and the size and position of the tongue, estimated by Mallampati and Friedman classification, with the findings of the overall polysomnography.
Materials and methods: The study involved 804 subjects who performed full-length polysomnography at the Sleep Medicine Center Split. Patients diagnosed with obstructive sleep apnea (OSA), based on the value of Apnea Hipopnea Index (AHI), which was ≥5, were recruited in the study group, while in the control group were recruited subjects who did not have obstructive sleep apnea, subjects with AHI <5. Subjects, except the full-length polysomnography records, had completed questionnaires for estimating excessive daytime sleepiness (Epworth Sleepiness Scale). Out of a total of 804 subjects, 242 subjects were examined by the specialist of otorhinolaryngology who determined upper respiratory tract abnormality assessed by the Mallampati classification and the size and position of the tongue evaluated by the Friedman classification.
Results: The results of this study showed that subjects with diagnosed obstructive sleep apnea had a greater excessive daytime sleepiness than those without obstructive sleep apnea (7.7 ± 4.9 vs 6.7 ± 4.6, P = 0.013). Furthermore, the results of the study showed that in the population of patients with OSA value of excessive daytime sleepiness determined by Epworth sleepiness scale (ESS) better correlates with AHI value than the obstruction of the upper respiratory tract estimated by Mallampathi or the size of the tongue classified by Friedman.
Conclusion: Based on the results of this study it was determined that a higher stage of obstruction of the upper respiratory tract and the size and position of the tongue, estimated by Mallampati and Friedman classification, are not necessarily associated with severe obstruction sleep apnea, which would mean that stage IV of Mallampati classification (range I-IV) does not indicate with high probability a higher degree of OSA, as stage I can not safely exclude the diagnosis of OSA.
