Abstract
Cilj istraživanja: Rak mokraćnog mjehura, kao sedmi najčešći zloćudni tumor, predstavlja globalni zdravstveni problem. Uz dobro poznate faktore rizika kao što su pušenje i utjecaj kemikalija, razna infektivna sredstva sudjeluju u patogenezi urotelijalnih tumora. Cilj ovog istraživanja bio je utvrditi postoji li ikakva povezanost virusne infekcije i raka mokraćnog mjehura u hrvatskih pacijenata. Materijal i metode: Uzorci biopsije od ukupno 55 bolesnika s različitim stadijima tumora mokraćnog mjehura su prikupljeni i analizirani. Iz uzoraka su izolirani DNA i proteini. Početna analiza uzoraka je bila putem DNA čip tehnologije, a zatim su korištene metode PCR, DNA sekvencioniranja i imunokemijske metode detekcije proteina–Western blot. Rezultati: Početno ispitivanje DNA čip-om na prisutnost virusa otkrilo je prisutnost Herpesvirus-a povezanog s Kaposijevim sarkomom (KSHV) u svakoj od tri nasumce odabrana uzorka biopsije. Prevalencija zaraze KSHV među ispitivanom populacijom dalje je analizirana KSHV-specifičnom lančanom reakcijom polimeraze (PCR) i imunokemijskim reakcijama. Reakcijom nested PCR – ugniježđenom lančanom reakcijom polimeraze, KSHV DNA otkrivena je u 55 % bolesnika. Zaključci: KSHV, također poznat kao humani herpes virus 8, je infektivni agens s već dokazanom ulogom u nastanku raka. Njegov onkogeni potencijal ima ulogu u nastanku Kaposijevog sarcoma te je također uključen u patogenezu nastanka dva limfoproliferativna poremećaja. Visoka prevalencija KSHV infekcije u našoj studiji pokazuje da KSHV može igrati ulogu u nastanku tumora mokraćnog mjehura i pokazuje potrebu za dodatnim istraživanjima.
Abstract (english)
Aim od study: As the seventh most common human malignancy, bladder cancer represents a global health problem. In addition to well-recognized risk factors such as smoking and exposure to chemicals, various infectious agents have been implicated as cofactors in the pathogenesis of urothelial malignancies. The aim of the present study was to assess the possible association of viral infection and bladder cancer in Croatian patients. Materials and methods: Biopsy specimens were collected from a total of 55 patients diagnosed with different stages of bladder cancer. DNA and proteins were isolated from tissue sample. The initial analysis of samples was through DNA chip technology and afterwards PCR, DNA sequencing and immunoassay detection of protein-Western blot were used. Results: Initial screening of DNA extracts for the presence of viruses on Lawrence Livermore Microbial Detection Array revealed Kaposi's sarcoma-associated herpesvirus (KSHV) in each of three randomly chosen biopsy specimens. The prevalence of infection with KSHV among study population was then examined by KSHV-specific polymerase chain reaction (PCR) and immunoblotting. By nested PCR, KSHV DNA was detected in 55 % of patients. Conclusion: KSHV, also known as human herpesvirus 8, is an infectious agent known to cause cancer. Its oncogenic potential is primarily recognized from its role in Kaposi's sarcoma, but it has also been involved in pathogenesis of two lymphoproliferative disorders. A high prevalence of KSHV infection in our study indicates that KSHV may play a role in tumorigenesis of bladder cancer and warrants further studies.
