Abstract Uvod: Lumbalni bolni sindrom (LBS) definira se kao bol, mišićna napetost ili nelagoda u području tijela između donjeg rebrenog luka i donje glutealne brazde sa širenjem u nogu ili bez njega. Predstavlja jedan od vodećih zdravstvenih problema u zapadnim industrijaliziranim zemljama. Nakon obične prehlade, drugi je najčešći razlog posjete liječniku obiteljske medicine te je drugi po redu uzrok onesposobljenosti uopće. Prevalencija lumbalnog bolnog sindroma kreće se između 60 i 85%, dok u bilo kojem trenutku 12-45 % odraslih osoba ima neku vrstu nelagode u donjem dijelu leđa. LBS možemo podijeliti na nespecifični i specifični bolni sindrom a s obzirom na tijek i trajanje dijelimo ga na akutni, subakutni i kronični LBS. Najprihvatljivija podjela s obzirom na uzrok je ona na mehanički lumbalni bolni sindrom, lumbalni bolni sindrom s radikulopatijom, patološki lumbalni bolni sindrom i psihološki prikriveni lumbalni bolni sindrom. Terapijske vježbe su jedan od najčešćih oblika liječenja LBS-a. Vježbe koje smo pretraživali su: vježbe jačanja, vježbe istezanja, stabilizacijske vježbe te koncept vježbanja po McKenziu.
Cilj rada: ispitati učinkovitost različitih programa vježbi na smanjenje boli i povećanje funkcije kod osoba koje boluju od kroničnog lumbalnog bolnog sindroma.
Metode: Ovaj sustavni pregled je obuhvatio pretragu stručne literature i znanstvenih članaka u periodu od kolovoza 2016. godine do svibnja 2017. godine koristeći PubMed, Google Znalac, Medline i PEDro baze podataka. Uspoređivala su se četiri različita intervencijska programa vježbanja te njihova učinkovitost na smanjenje boli i povećanje funkcije kod osoba s KLBS. Intervencijski program vježbanja obuhvaćao je vježbe jačanja, vježbe istezanja, stabilizacijske vježbe te koncept vježbanja po McKenziu.
Rezultati: Obuhvaćeno je 20 članaka različitih autora koji su bili fokusirani na smanjenje boli i povećanje funkcije kod osoba koje boluju od kroničnog lumbalnog bolnog sindroma. Ukupno 1611 pacijenata u rasponu od 18 do 70 godina sudjelovalo je u ovom sustavnom pregledu, bez jasne raspodjele na muške i ženske sudionike. Vježbe jačanja i vježbe istezanja nisu pokazale pozitivne rezultate u smanjenju boli i povećanju funkcije kod osoba s KLBS. Vježbe stabilizacije u kombinaciji s dorzalnom fleksijom stopala te sa standardnim vježbama jačanja po Reganu i Michellu pokazale su se kao superioran oblik vježbanja u odnosu na ostale intervencijske metode s kojim su bile uspoređene. McKenziev koncept vježbanja u usporedbi s Williamsovim konceptom vježbanja te vježbama na lopti pokazao je superioran utjecaj na smanjenje boli i povećanje funkcije kod osoba s kroničnim lumbalnim bolnim sindromom.
Zaključak: Vježbe jačanja i vježbe istezanja nisu pokazale superiorniji učinak na smanjenje boli i povećanje funkcije kod osoba s kroničnim lumbalnim bolnim sindromom dok su se stabilizacijske vježbe te McKenzijev koncept vježbanja pokazali kao superioran tretman za smanjenje boli i povećanje funkcije u odnosu na ostale intervencijske metode.
Abstract (english) Background: Lumbar Pain Syndrome (LPS) is defined as pain, muscular tension or discomfort in the body area between the lower rib arch and the lower gluteal groove with or without the spread of the leg. It represents one of the leading health problems in Western industrialized countries. After a common cold is the most common reason for visiting a family doctor and is the second most common cause of disability at all. The prevalence of lumbar pain syndrome ranges between 60% and 85%, while at any given moment 12-45% of adults have some kind of discomfort in the lower back. LPS can be divided into non-specific and specific pain syndrome and, given the course and duration, we divide it into acute, sub-acute and chronic LPS. The most acceptable division due to the cause is that of mechanical lumbar pain syndrome, lumbar pain syndrome with radiculopathy, pathological lumbar pain syndrome and psychologically concealed lumbar pain syndrome. Therapeutic exercises are one of the most common forms of LPS treatment. The exercises we analysed were: strengthening, stretching , stabilization and McKenzie training exercise.
Objective: to examine the effectiveness of different exercise programs to reduce pain and increase the function of people with chronic lumbar pain syndrome.
Methods: this systematic review included a search of professional literature and scientific articles from August 2016 to May 2017 using PubMed, Google Scholar, Medline and PEDro database. Four different intervention training programs were compared and their effectiveness in reducing pain and increasing the function of persons with CLPS. The exercise program included strengthening exercises, stretching exercises, stabilization exercises, and McKenzie training.
Results: 20 articles of various authors were included, focused on reducing pain and increasing function in people with chronic lumbar pain syndrome. A total of 1611 patients in the range of 18 to 70 years participated in this systematic review without a clear distribution of male and female participants. Strengthening and stretching exercises did not show positive results in reducing pain and increasing function in persons with CLPS. Stabilization exercises combined with the dorsal flexion of the foot and with the standard program of Regan and Michelle exercises proved to be a superior form of exercise compared to the other interventional methods. McKenzie's exercise concept compared to Williams's exercise concept and ball exercises show superior effect on pain reduction and increased function in people with chronic lumbar pain syndrome.
Conclusion: strengthening and stretching exercises did not show a superior effect on pain reduction and increased function in people with chronic lumbar pain syndrome while stabilization exercises and McKenzie's workout concept demonstrated superior treatment for pain reduction and increased function compared to other intervention methods.
